Yes because you don't hold one in your hand it's mounted on the ground and they do fly back quite a bit, but they're not in the game anyway so...

I am experienced with running servers with custom game modes, I have run a lot of Garry's Mod servers before where we used our own bespoke scripts rather than doing what 99% of servers owners do where they just copy and paste and say they made it Perhaps I could set up an Identity server that is custom and differs from the vanilla experience but I'm not certain if this is possible or even if there is scripting support at all

Newton's Third Law: Every action has an equal and opposite reaction, apply this to guns and you shooting someone in the chest and them falling backwards means that you would also fall backwards, because if the "reaction" is powerful enough to knock the guy getting shot over then it will also do this to you because both forces will be equal. Have you ever seen an old artillery cannon? Like a WW2 era one? Notice how they have wheels and when they shoot they roll quite far back that is what would happen if a gun was powerful enough to knock someone over, imagine the cannon as the shooter and that's what would happen. But no gun you hold in your hand is powerful enough to do this and the reason you imagine people would get knocked over is because of stunts in films and TV series etc. Hope I helped you to understand

I think it would be a cool idea to have scars and other disfigurements in the game that you would acquire after you create your character. So say you were attacked by someone with a knife and that cut you down your arm after you have healed and what have you there will be a scar there to tell the tale or say you were in a fire and you got burnt wherever you got burnt would have the blemish after you have been healed. Now of course a lot of players wouldn't want their character to have this kind of deformity, so I think if you wanted to get rid of the disfigurement you could go to the hospital or clinic to have cosmetic surgery which is one way of keeping immersion and not breaking the fourth wall.
